Re: THE ALERT NOTICE
The following is being
provided to you to assist with calls that you may be receiving from members
regarding the instructions provided on the recently mailed ALERT Notice
from the CCWIPP. Due to the sheer volume
of calls, both credit reporting agencies, Trans Union and Equifax, have
provided alternative methods for CCWIPP participants to contact them.
Equifax
Alternatively, participants may put an ALERT notice on their credit file by mail, fax or e-mail. The request should indicate “IMPORTANT ALERT” at the top.

2/ E-mail a request to fraud.assistance@equifax.com indicating
the full name, current address, SI Number (optional), date of birth and two
pieces of valid identification (at least one piece must have a signature and
the current address). The pieces of ID
must be scanned and provided as an attachment to the e-mail.
Trans Union is very difficult to contact by telephone, therefore, this
agency has established a web site www.tuc.ca/pen with a
link that provides instructions to download the following forms:

Once the forms are completed they
may be mailed or faxed to Trans Union

There is no fee charged by
either Credit Reporting Agency.
With respect to contacting
Human Resources Development Canada, to report a stolen SI Number, we have been
informed that this is not required. HRDC
has confirmed that it should be contacted only if identity theft involving a
social insurance number, is confirmed.
